Swing Bed Program
What is Swing Bed?
PMH Medical Center Swing Bed Program is designed to provide patients with individualized in-hospital care and physical rehabilitation to assist them to reach a maximum level of function. This program is designed for patients who are discharged from acute care, but need temporary additional supportive care that cannot be provided at home.
Through a combination of rehabilitative therapies, medical care, and nursing care, patients gain the function they need to achieve their maximum level of independence. This program bridges the gap from acute hospitalization to home.
Who qualifies for a Swing Bed Stay?
Medicare and most insurance companies cover swing bed stays as a post acute care rehabilitation. Admissions can come to PMH Medical Center from any hospital, including PMH. Medicare patients must have spent three consecutive midnights as an inpatient in acute care within the last 30 days. Private insurance and managed care rules vary. We can help determine if you qualify for Swing Bed Care.
Referrals to PMH Medical Center Swing Bed Program can come from a number of sources. Examples include:
- Your physician
- Your discharge planner
- Insurance case managers
- You may also call us directly to work with doctors or others providing care to determine if you are eligible to the program.
Physicians: Hospitalists manage each patient and are available 24/7 if a need arises. Additionally, there is a physician ONSITE that is available to provide immediate care in the event of an emergency.
Nurses: Nurses assist with individualized daily care and ensure a smooth transition into the program. Nurses also provide medication, wound care administration, and teaching as directed by the hospitalist.
Rehabilitation: Physical Therapy, Occupational Therapy, and Speech Therapy work with your specific needs to help you reach physical goals to maximize independence.
Dietitian: A Licensed dietitian completes an evaluation to determine nutritional needs and provide recommendations. They also give nutritional education to patients and caregivers to meet the nutritional needs of our clients.
Social Worker: Our social worker provides psychological support, discharge planning and care coordination. The social worker also assists you in identifying and coordinating care needs and resources for your discharge, even assisting with transportation arrangements for follow up appointments during and after your swing bed stay.
Pharmacy: Pharmacist is available 24/7 for consultation.

Each patient makes progress at an individual rate. The Interdisciplinary Team monitors and evaluates your progress and goals until you are ready for discharge. You will be assisted by the team to ensure a safe discharge.
Our Physical and Occupational Therapy Team may also educate you and your family or caregiver on safe body mechanics and adjustments that may be made in your home to ensure your success at home with your activities of daily living. Home Health, Outpatient Physical and Occupational Therapy referrals may occur if indicated at time of discharge.
Occasionally it is necessary to remain hospitalized beyond the acute stage of a surgery or illness.
